Mind Over Matter Nursing Inc., doing business as Mind Over Matter Mental Health Group, operates mindovermattercare.com. This policy explains information handling on our public website. Protected health information received or maintained for patient care is also subject to our Notice of Privacy Practices and applicable law.

Information you choose to provide

When you contact us, request an appointment, or submit a referral, we receive the information you choose to send, such as your name, contact details, message, scheduling preferences, and any patient or referral information included in your submission. Provide only information needed for your request, and only information you are authorized to share.

We use inquiry information to respond to you, discuss your request, arrange appropriate next steps, and maintain the communications needed to do so. We do not use inquiry details for unrelated marketing or sell them. If you become a patient, information needed for care, scheduling, billing, or required records is handled under our patient privacy notice and agreements.

General website forms and ordinary email should not be used for emergencies or unnecessary sensitive medical information. Contact the practice for the appropriate secure channel for records and clinical messages.

Website hosting, cookies, and analytics

The site is hosted by Squarespace and uses Squarespace analytics and Google Analytics. These services may process technical and usage information such as IP address, browser and device information, referring pages, pages visited, dates and times, and interactions with the site. Cookies and similar technologies may support site operation, security, and traffic measurement.

We use website traffic information to understand and improve the site. This technical collection is separate from information you deliberately submit to contact the practice. Service providers process data under their applicable terms and privacy policies. You can use browser settings to limit or delete cookies. Blocking cookies may affect site features. You can also use Google's Analytics opt-out browser add-on. The site does not currently change its technical or analytics collection in response to a browser's Do Not Track signal. Third-party services may receive usage information when their content loads and may recognize your browser across sites under their own settings and policies. We do not use inquiry information for cross-context behavioral advertising.

Service providers and disclosures

Website hosting, form processing, communications, scheduling, and related service providers may process information to support your request and operate these services. Squarespace supports the public website. The practice also uses or links to systems such as SimplePractice, Spruce Health, and Headway for relevant patient services. Opening a third-party site or portal is subject to that service's privacy practices as well as any applicable duties to the practice and patient.

We do not sell or rent information submitted to the practice. We may disclose information with your authorization, to service providers as needed for their work, or when legally required or permitted to protect rights, safety, security, and the integrity of our services. Disclosures of protected health information must also meet applicable health-privacy requirements.

Retention and security

We retain information as reasonably needed to handle requests, operate the practice, and meet applicable recordkeeping, legal, and security obligations. Information that becomes part of a medical or billing record may need to be retained even if you request deletion. We use reasonable safeguards, but no website, email system, or internet transmission can be guaranteed completely secure.

Your choices and requests

You may choose not to submit information through the website and contact us by phone instead. We may need to verify your identity and will respond as required by applicable law. Some information must be retained or cannot be changed; we will explain applicable limits. The public website is intended for adults seeking information and for parents or guardians seeking care for a minor. Children under 13 should not submit personal information through general website forms. A parent or guardian who believes a child has done so may contact us for assistance.
