If you plan to use insurance through Headway, choose the Headway option. If you are private-pay or want to request directly through the practice, choose the private-pay SimplePractice request option.
I’m already a client. Should I use the booking buttons?
Existing clients should use the client portal sign-in so appointments, messages, and forms stay connected to the correct chart.
I’m a therapist, PCP, or community partner. Where do I refer?
Use the referral page. It is built specifically for provider referrals and includes instructions for submitting referral information securely or by fax.
Can I use the website forms for a crisis?
